Duration: 5 months Convertible, Start from 01 Apr 2024
Working Location: Bukit Batok
Working hours: 08.30am – 5.45pm (Monday to Thursday), 08.30am – 5.30pm (Friday) & (One Saturday per 4 weeks from 08.30am – 12.30pm)
Job Responsibilities
- Perform social and financial assessment of patients and make recommendations.
- Provide counselling to patients and their families and lead in family conferences.
- Liaise with relevant organisations for collaboration and networking in care of the elderly.
- Attend to the social needs of destitute patients.
- Participate in a multi-disciplinary team on patient-care.
- Conduct groupwork with the elderly patients or their caregivers.
- Implement initiatives and support programmes that enhances the care and support of elderly patients and their caregivers within the community
Job Requirements
- Degree and postgraduate diploma in Social Work
- Must have an interest to work with the elderly in a healthcare setting
- Ability to communicate with vernacular languages speaking clients. (in order to provide psychosocial, financial support and assistance to Vernacular Languages Speaking Clients.)
