- Performed medication reconciliation, to identify medication(s) taken prior to admission, drug related problems leading to current admission and documented it
- Handling stocks and order supplies of medicines in unit of dose to the ward
- Processing medication orders and packing in accordance to SOP
- Responded to queries or drug information to patients, caregivers, healthcare professionals, students and/or public
- Performed medications counselling, inhalers technique counselling, warfarin counselling to the patients and/or caregivers
- Review and process medication orders and provided pharmaceutical care plan to the patient
Requirements:
- Recognized Degree in Pharmacy
- 3 years and above of relevant experience
- Able to perform shift duties
