Job Description:
- Work closely with patients including intake interviews, financial assessment and assistance, initial family assessment for care arrangement cases
- Liaise with Medical Social Worker to facilitate effective delivery of social work services
- Process Medifund applications and submissions at CMMAS to ensure compliance
- Provide support to patients and family members at reception counter
- Follow up on the utilization and disbursement of various applications of funds
- Keep track of departmental funds and prepare monthly report
Requirement:
- Diploma in Social Sciences / Social Work / Applied Psychology
- 2 to 3 years of relevant experience in local social or community work setting
