- To provide bio-psychosocial assessment and intervention in management of patient care that meets standard of practice.
- Provide holistic and end-to-end case management support as well as appropriate level of care and emotional support to patients and families through assessment, care planning, interventions, and evaluation of care.
- Represent case in case conference and represent department in inter-department or inter-agencies workgroups to discuss common cases independently and/or complex cases with guidance or supervision to formulate professionally endorsed treatment intervention plans.
- Identify service gaps in response to clients’ needs and make recommendations to improve service provision of programmes.
- Participate and implement programmes, as well as to assist in the development of programmes
- Engage in learning opportunities, (e.g. participate in Communities of Practice) to develop skills and capabilities.
Requirements
- Degree / Diploma in Nursing/Allied Health/Psychology/Counselling/ Social Work/Sociology is preferred.
- Professional Registration/Licensure
- Registered with relevant professional authority when appropriate.
- At least 3 years in case management / care coordination within Social Service or Healthcare sector with Elderly or Persons with Disability.
- Good social case work intervention skills

- Self-awareness and ability to use self in facilitating change
- Proficient in Microsoft applications
