RESPONSIBILITIES

Responsible for case work management involving systemic assessment and intervention for the well-being of patients and family members.
Performs referrals to other service providers.
Conducts home visit.
Works closely with multi-disciplinary team members.
Leads and/or support projects and programmes.
Attends training and case work supervision.
Any other related duties as directed by the said reporting officers.

QUALIFICATIONS

Degree in Social Work or a Graduate Diploma in Social Work.
Registered with Singapore Association of Social Work.
Able to communicate with Mandarin-speaking patients.
Experience in means testing and healthcare setting is advantageous but fresh graduate may apply.
