Job Description:
- Conduct home visits to patients with chronic illnesses or in palliative care.
- Conduct nursing assessments on patients during visits.
- Coordinate with doctors for medical assessments and subsequent follow-up visits.
- Monitor medication usage and ensure compliance with prescribed regimens.
- Perform nursing procedures including wound dressing, urinary catheter insertion, nasogastric tube insertion, and tubing care when necessary.
- Develop and implement nursing care plans, introducing new management strategies as needed.
- Educate caregivers and families on essential nursing procedures to facilitate ongoing care at home.
Requirement:
- Minimum Diploma in Nursing
- Candidates with 1 to 3 years of ward/nursing home
- Palliative / geriatric experience preferred
