Job Description:
- Assess needs of seniors with care/service needs and formulate care plans for seniors with care/service needs
- Assist in the development and implementation of programmes for seniors through data collection and the conduct of basic analysis
- Coordinate care for the seniors with relevant stakeholders
- Documentation of case notes
- Facilitate timely and appropriate service referral and provision to seniors
- Participate in case discussions with external service providers
- Provide case management services
- Perform needs assessment of seniors living in the community
- Prepare data for submission to relevant authorities
Technical Competencies:
- Proficient in MS Word, Excel and PowerPoint.
- Good report writing skills.
- Able to meet deadlines.
- Administrative skills
- Able to engage stakeholders to develop solutions in community work
Core Competencies:
- Able to work independently and in a multidisciplinary team.
- Good interpersonal and communication (oral and written skills).
- Good knowledge about the community resources.
Preferred achievements/characteristics:
- Preferably a Graduate Diploma in Social Work or Degree in Social Work
- Preferably 2 years of experience in Social Service Sector or related field
